INDIVIDUALS RIGHTS AND RESPONSIBILITIES

As an individual/responsible party of Empowerment and Options, LLC, you have the right to:

  • Receive a copy and an explanation of your rights and responsibilities by a staff member at the time of your first visit with Direct Support Professional (DSP).
  • Receive a copy and an explanation of your rights and responsibilities prior to the delivery of services.
  • Expect that all Empowerment and Options, LLC staff will act in accordance with this and will interact with individuals and family accordingly.
  • Empowerment and Options, LLC employee conduct and document periodic reviews of your rights and responsibility for continued understanding.
  • Be informed of all information relative to the services rendered to you by Empowerment and Options, LLC employees.
  • Have all your property treated with respect.
  • You have the right to voice any grievances regarding the services and care being delivered or regarding the lack of respect for property by anyone who is providing services on behalf of Empowerment and Options, LLC. Empowerment and Options, LLC will document the complaint or grievance and the resolution. You have the right to contact the Executive Director at any time when you feel you want to voice a complaint. The Executive Director will then record the complaint/concern and investigate and devise a plan of action.
  • Be informed in advance about the service plan and of any changes in your services before the change is made.
  • You have the right to have your individual records kept confidential and filed appropriately.
  • You have the right to have your records disclosed or released when written authorization is provided by you or your conservator.
  • You have the right to be informed and advised prior to services rendered of the charges (if any) that will be incurred as a result of having Empowerment and Options, LLC services.
  • You have the right to privacy and privacy being respected and properly protected.
